PASSPORT REQUIREMENTS FOR FIRST TIME APPLICANTS

1. Birth Certificate (BC) in Security Paper (SECPA) issued by the National Statistics Office (NSO) or Certified True Copy (CTC) and photocopy of BC issued by the Local Civil Registrar duly authenticated by NSO and document indicating full name, date and place of birth and citizenship (e.g. valid employment's ID, voter's affidavit, NBI clearance and driver's license).

IN CASE OF NO BIRTH RECORD

If born after 1950

-In case of birth certificates with delayed registration, submit authenticated BC and supporting documents indicating date, place of birth and citizenship.

If born on or before 1950

b. Certificate of Non-availability of Birth Record from the NSO.

c. Joint Birth Affidavit signed and executed by two (2) disinterested persons.

d. Baptismal Certificate or Certified True Copy of Voter's Affidavit from COMELEC or any public document indicating date and place of birth and citizenship.

2. Three (3) copies 4.5 cm x 3.5 cm new photo (colored with plain white background). Photo should show applicant in decent attire with collar.

  • Personal appearance except when the applicant is:

- 8 years old and below;
- 65 years old and above; or
- mentally or physically incapacitated

 

ADDITIONAL REQUIREMENTS

A. For married women who chose to adopt surname of husband

Marriage Contract (MC) in Security Paper issued by the National Statistics Office (NSO) or CTC issued by the Local Civil Registrar duly authenticated by NSO.
* For married women opting to use maiden name, submit MC & affidavit indicating that she has not use her Married name.

B. For women who obtained an annulment or were divorced by foreign husband

Authenticated copy and photocopy of the first page and the dispositive portion of the judgment granting divorce or annulment.
* Certified true copy and photocopy of the first page and the dispositive portion of the judgment granting divorce or annulment, authenticated by the Philippine Embassy/Consulate when the divorce was obtained.
* Annotated Marriage Contract (MC) in Security Paper (SECPA)

C. For fiancées and spouses of foreign nationals

Original and photocopy of Commission on Filipinos Overseas (CFO) Guidance and Counseling Certificate of Attendance.

D. For minors (below 18 years old)

- Personal appearance of either parent.
- If minor is not traveling with either parent, submit an original and photocopy of DSWD Clearance and Affidavit of Support and Consent indicating the name of the traveling companion and relationship to the minor.
- If both parents are abroad, such an affidavit must be authenticated by the nearest Philippine Embassy or Consulate General.
- If both parents are abroad, person applying in behalf of the minor must submit Special Power of Attorney authenticated by the nearest Philippine Embassy or Consulate General.
- Passport and photocopy of the passport of person traveling with the minor.

E. For adopted children

- Authenticated copy and photocopy of the dispositive portion of the Court Order on adoption.
- Authenticated copy of original and amended Birth Certificate.
- DSWD Travel Clearance

F. For illegitimate children born after August 13, 1988

- Personal appearance and written consent of mother
- DSWD Clearance if minor is traveling with Father/other persons.

G. For Legitimated Children due to subsequent marriage of parents

- Authenticate BC with annotation of the amended surname of the child and MC of parents.

H. For Muslim applicants

- For those whose births were registered, follow requirements for new applicants.
- For those whose births were not registered.
- Certified True Copy of Late Registered Birth Certificate from the National Statistics Office.
- Original and photocopy of Voter's Affidavit or NBI Clearance or other supporting documents indicating date and place of birth and citizenship.
- Certificate from the Office on Muslim Affairs for Muslim convert
- For Muslim converts, submit annotated BC in Security Paper (SECPA)
- Court Order to change name.
- Shari'ah Court Order.

  • Additional supporting documents may be required when necessary

 

 REQUIREMENTS FOR PASSPORT RENEWAL

  • Renewal Requirements

* 3 copies of picture
* Passport Size 1.77" x 1.37"
* 3cm. from head to chin
* Colored with plain white background
* Decent attire with collar and sleeves
* Studio taken
* Most recent
* Identical and frontal shot
* Clear, no shadow
* Proportionately taken
* Not Allowed:
* Earrings or long hair for male
* Eyeglasses
* Signature / writings (front and back)
* Computerized, fotome, Polaroid
* Same picture with old passport.
* Original Passport- Green
* If you're a brown passport holder, there will be a personal appearance at the DFA but with assistance, and you should submit any of the following documents with complete middle name (original & photocopy)
* Birth Certificate
* Baptismal Certificate
* Old SSS E-1 form
* GSIS policy
* Driver's license
* PRC ID
* Voters Affidavit or CTC from COMELEC
* Income Tax Return
* Transcript of Records
* NBI Clearance
* Seaman's Book
* Marriage Contract
* Photocopy of passport from pp. 1-4, last page, and the pages showing latest BID Departure and Arrival stamps.

  • For Seamen

Seaman's book is required for all seamen, because departure and arrival stamps are indicated on its pages.

  • For Married Women
  •  If you want to adopt your husband's surname in the passport

Marriage Contract in Security Paper issued by NSO or Certified True Copy of MC authenticated by NSO East Ave. if newly married (original & photocopy).

  • For Minors
  • Traveling w/ both parents
    * Original copy and photocopy of the passports
    * Photocopy of Birth Certificate of Minor
    * If w/ father only:
    * Marriage Certificate
    * Affidavit of Support and Consent from the mother (8 yrs. old and below)
    * Original ID w/ signature or passport of mother

    Traveling w/ guardian
    * Original copy and photocopy of the guardian's passport
    * Photocopy of Birth Certificate of Minor
    * DSWD Clearance (original & photocopy)
    * Special Power of Atty. indicating the name of the guardian, authenticated by the nearest Phil. Embassy if executed abroad.
    * Affidavit of Support & Consent from the parent indicating the Traveling Companion and the relationship with the minor, authenticated by the Philippine Embassy if executed abroad.
    * Original ID/Certified True Copy (CTC) authenticated by Phil. Embassy of parent's ppt (whoever executed the SPA & Aff. Of Suppt. & Consent)

    Traveling w/ parent but the application is being renewed by guardian
    * Photocopy of Birth Certificate of minor
    * Special Power of Atty. indicating the name of the guardian authenticated by the nearest Phil. Embassy, if executed abroad.
    * Affidavit of Support & Consent from the parent indicating the name of the traveling companion & relationship to the minor.
    * Orig. ID w/ picture & sign. or orig. ppt of the guardian.
    * CTC of parent's passport authenticated by the nearest Phil Embassy, if parent is in abroad
  • Traveling w/ guardian but the application is being renewed by others
    * Photocopy of Birth Certificate of minor
    * Orig. & photocopy of the passport or CTC of companion's passport authenticated by the nearest Phil. Embassy or Consulate General if he/she is in abroad.
    * Special Power of Atty. to the person who will renew the passport from the parent authenticated by the nearest Phil Embassy or Consulate, if executed abroad.
    * DSWD Clearance (original & photocopy)
    * Affidavit of Support & Consent from the parent indicating the traveling companion and the relationship to the minor.
    * Orig. ID or passport of parents.
    * Orig. ID w/ picture & signature of person who will renew the passport.

For Illegitimate Children

- Photocopy of Birth Certificate
- Original & photocopy of the passport of traveling companion
- If traveling w/ father, Affidavit of consent from the mother & original ID or  passport of the mother, DSWD clearance.

Reminders

* Affidavit of one & the same person if there's a discrepancy in parents' first name   (passport of parents vs. birth certificate of minor)
* Possible appearance of minor if there's a big discrepancy in facial features

 

REQUIREMENTS FOR REPLACEMENT OF LOST PASSPORT

* If already expired, submit notarized Affidavit of Loss. No penalty fee of P200.00
* If still valid, submit Police Report and notarized Affidavit of Loss. There will be a 15-day clearing period prior to the processing of application for a new passport.
* Personal Appearance.
* Birth Certificate (BC) in security paper and supporting documents
* Three (3) copies 4.5 cm x 3.5 cm new photos (colored with plain white background). Photos should show applicant in decent attire with collar.

The following documents may be necessary on a case to case basis and should be provided when required. They may also be substitutes for lack of other documents but should not be construed as sufficient when primary documents are unavailable.

REMINDERS

* Applicant should file his application where he is physically present.
* Declare lost, valid, or expired passport to avoid delay in the issuance of a new one.
* Passport unclaimed after one (1) year will be cancelled per Memorandum Circular 33-88.
* Check all data in the passport upon release. Passports found to have scripting errors will be replaced gratis if presented for correction 6 months from date of release.
